Long Term Condition Reviews
If you live with one of the following long term conditions you will be offered an annual review in your birthday month which will include an appointment with a clinician to take bloods and take physical measurements such as your weight and blood pressure. You will then have a follow up appointment with the most appropriate member of the team which will include your annual medication review.
- High blood pressure
- Heart disease or stroke
- Diabetes or pre-diabetes
- Serious mental illness, dementia, or a learning disability
- Asthma or COPD
- Rheumatoid arthritis
- Coeliac Disease
- Severe Frailty
- Post-Bariatric Surgery
NHS Health Checks
You can make an appointment for a NHS Health Check with one of our Health Care Assistants. During these appointments the Health Care Assistant will discuss general health issues and check your blood pressure, weight and urine sample. These checks are for 45 years and over and are designed for both male and female patients.
How can you book an appointment for an NHS Health Check?
Request a consultation by phone - call 01223 423424 during our open hours. We will check if you are eligible to be booked in. You will also receive a text message or a telephone call when you are eligible for this and then this can be booked in. When we invite you in, we will send you a text message if you have a mobile phone with a booking link attached for you to book a suitable time and date that suits you.

First Contact Practitioner - Physiotherapist (FCP)
Our FCP's are physiotherapists with enhanced skills in the assessment and diagnosis of musculoskeletal injuries and conditions. These include;
- All soft tissue injuries, sprains, strains or sports injuries
- Arthritis
- Possible problems with muscles, ligaments, tendons or bone
- Spinal pain
- Spine-related pain in arms or legs, including nerve symptoms
- Changes to walking
- Post-orthopaedic surgery/fractures
During your appointment you can expect your FCP to assess you, diagnose what is happening, provide expert advice on how best to manage your condition, and refer you on to specialist services if necessary.
You do not need to see a GP in order to access our FCP service - you can refer yourself directly!
